Consent Form for the Processing of Special Categories of Personal Data

By agreeing to the "Consent Form for the Processing of Special Categories of Personal Data", I, the data subject, hereby authorize LLC "KENARI" (I/N: 404392224; Legal Address: Georgia, Tbilisi, Didube district, Lubliana st., I lane, N4) to process my special categories of data, specifically data concerning health, for the purpose of providing services.

The processing of personal data concerning health refers to the processing, based on the consent of the data subject, of information related to the visual acuity (basic acuity, number) of the data subject when purchasing contact lenses and/or optical glasses from the company LLC "KENARI”, in order to provide services to the customer.

Based on this consent, personal data concerning my health, specifically information about my visual acuity (basic acuity, number), will be processed as information about me, the data subject.

I am informed that the processing of personal data concerning health is carried out based on the written consent of the data subject, specifically in accordance with Article 6 (1) of the Law of Georgia “ON PERSONAL DATA PROTECTION”. Therefore, I am not legally obliged to provide the company with personal data concerning health; however, if I do not provide this information, LLC "KENARI" may be unable to offer services to me.

By signing this form, I certify that my consent to the processing of personal data concerning health by LLC "KENARI” is informed, specific, and unambiguous.

Furthermore, I am informed that I may withdraw my consent to the company at any time by sending a message to info@kenari.ge. The withdrawal of consent does not affect the lawfulness of personal data processed based on that consent.

I understand that my personal data concerning health will be processed by LLC "KENARI" solely for the purpose of providing services to me prior to placing the order. After this, my personal data concerning my health, specifically my visual acuity (basic acuity, number), will be stored in accordance with the company's privacy policy for a period of three (3) years.

I confirm that I have received complete and comprehensive information about my rights as a data subject defined by the Law of Georgia “On Personal Data Protection”. In particular, I understand that LLC "KENARI” is obliged to inform me whether personal data concerning me is being processed, if i request such information, as well as the grounds for and purpose of that processing. I also have the right to access my personal data and obtain copies of such data free of charge, to rectify, update, and/or complete my data, and to request the termination of processing, erasure, or destruction of my data, except in exceptional cases defined by law.
